CANCELLATION POLICY

We are dedicated to providing you with the best quality of care and therefore do not double or triple book time slots . Each counseling session is reserved for you and you alone. Due to this policy, we ask that if you are unable to attend your session to notify us as soon as possible and no later than 24 hours in advance. Late cancellations (less than 24 hours before your appointment) will be charged a fee of $30, no shows will be charged a fee of $50 these fees are not billable to your insurance and must be paid out of pocket prior to rescheduling. 

DOCUMENTATION FEES

Any documents that are a part of a typical session course will be available at no cost to you such as Work or School Excuses, Superbills, or Schedule of Appointments.

Other documentation can be provided, such as attendance of therapy letters, summaries for court, treatment recommendations for IEP, etc will be charged a fee and will require a minimum of 48 hours or more to complete. 

The standard fee for phone calls between counselor and the client will be $15.00 per 15 minutes. Please note that a telephone conference cannot be billed to insurance. 

COURT OR DEPOSITION POLICY

We require a request to appear for court at a minimum 2 weeks prior notice in writing. Bringing in a counselor is not always the best option and testimony provided may harm your case instead of helping. Courtroom testimony/depositions will be billed at the fee stipulated in the counselor’s individual declaration of practice and procedures and must be paid two weeks in advance. These fees cannot be billed through insurance. Unpaid balances will be submitted to medical collections after 45 days from date of service.
